MEDICAL HOMEBOUND: FOR ALL STUDENTS(REGULAR AND
SPECIAL EDUCATION STUDENTS (GREEN FORM)
1. How long does a student have to be ill to be eligible for homebound services?
The student’s illness has to be of at least 4 weeks duration.
2. When can homebound start for the student?
The student can start a medical homebound as soon as the completed paper work is done. The physician completes the medical portion of the form, the parent completes their section, it is then to be returned to the school for class schedule and the principal or designated signature.
3. Where should the school forward the completed homebound form?
The completed form is sent to: Attention: Homebound/Student Services, Central Office.
4. Is the green homebound form only for medical illness?
Yes
5. Where do I get the medical homebound forms?
Call the student services office at 462-4052 and forms will be placed in the school mail the same day. (Schools also have a supply on hand.)
6. What are the conditions required to qualify a student for medical homebound?
The student must have an evaluation by a physician. The evaluation by the physician must indicate that the child has an illness or injury requiring hospitalization or homebound care as part of medical treatment. The Department of Education requires a minimum of four weeks disability/confinement to be eligible for homebound. The parent can apply for service as soon as the diagnois is made which qualifies the student for homebound instruction. Do not wait until the student has missed four (4) weeks of school prior to applying for homebound service if the child is physically ready to be receiving homebound instruction.
7. Does “pregnancy” qualify a student for homebound instruction?
No, unless the physician’s diagnois is a specific complication
due to the pregnancy.
Special Education Homebound (non-medical)
The green homebound form is to be used for medical homebound only.
Special Education Students placed on homebound for reasons other
than health need to have a special education conference with your school
representatives, parent(s) and the Covered Bridge Representative.
Covered Bridge will then complete the necessary paper work and forward
to the Student Services Office for homebound to be processed. If
not medical, this is the only way a special education student will be placed
on homebound. The paper work must go through the Covered Bridge office.
